 Okay, that adds a lot more of the cases I'm seeing. Still, 3 are missing:

 OP_TIMEOUT_5: transfer5m,572
 OP_UNKNOWN_1: transfer50k,563 and transfer50k,581
 }}}

 Any idea why these are still missing?

 (Note how we'll later be able to re-use these fixes to your script to
 further classify the failures! So even if this bug hunting might seem
 annoying at the moment, we might benefit from it later on.)
